1.How to recognize a child with Tongue Adhesions
– Depending on the degree of adhesion to the tongue brake more or less, the symptoms of tongue brake adhesion:
- The tongue brake line is short sticking to the tip of the tongue
- The tip of the tongue is difficult to reach the upper molar socket
- Tongue movements limited back and forth
- Tongue tip does not protrude more than 1-2mm from mandibular incisors
- Tongue tip is notched or heart-shaped when pushing the tongue forward
- The incisors are tilted or open
- Have problems with pronunciation (Sounds: “t”) , “d”, “z”, “s”,” th”, “n”,” l.”

2.How will Tongue Adhesion affect the baby?
– Baby’s sucking
– Having problems with pronunciation
– Affecting oral hygiene (such as difficulty licking lips or using tongue to sweep food on teeth) in older children
– Feeling of entanglement in tongue area
– Difficulty licking ice cream, playing musical instruments (wind), or kissing in older children
3.Indications and timing of Surgery
Depending on the level and condition of the child, the doctor will have a treatment plan suitable for each case, usually there will be 3 cases:
1/ If the tongue tie does not cause any problems in the neonatal period, follow-up is the best option.
2/ If other causes of difficulty suckling have been ruled out, surgical removal of the tongue will be the treatment of choice.
3/ If the child has problems with pronunciation, after considering and evaluating the cause of the tongue brake, surgery will be indicated.
4. Frenotomy
– Frenotomy (Frenulotomy) surgery: A simple laryngectomy for cases where only the membrane is attached, because the procedure is simple, it can be done with local anesthesia or no anesthesia.
– Frenuloplasty: Surgery to cut and shape the tongue brake for complicated cases of tongue adhesions. This procedure requires anesthesia.
